I was contacted through a recruiting agency. A person from HR had a brief conversation, followed by which I had a phone screen. It was a coderpad exercise, where I was constantly supposed to verbally comment about what it was that I was trying to do, without access to the Internet or any help tools. I knew I was being watched, but had lack of familiarity with the coding environment and its shortcuts; I do not ever code while verbally describing what I am trying to do, and knew that I was not allowed to go in the wrong direction, and retrace, like in real life coding... Asked for time to do it offline after the interview; did it in my own coding environment with access to vim, and completed it in ten minutes. Sent a GitHub link with the solution to the interviewer by email. Completely unreflective of real-world programming problems, and if it DID reflect the environment of the organization, then it gave a bad impression. The punchline of course, is that I did not receive a rejection email from the firm. They instead seem to follow the policy of staying silent, aka ghosting you.
