Accountability is selective — some employees do very little yet face no consequences, while others carry the weight. Leadership is widely seen as untrustworthy; feedback is often insincere and retaliatory, so staff avoid speaking openly. A pervasive lack of trust; people have “corridor conversations” rather than raising issues formally. Unconscious bias is severe and ignored, damaging careers and morale.
