As part of a true interdisciplinary team you'll be able to closely manage the co-morbid conditions of patients at risk for repeated hospitalizations and at risk for living in long term care facilities. Patients are enrolled into the program and have a complete interdisciplinary team who work together to create personalized plans of care for patients. You will not be rushed and limited to a typical 15 minute visit and care provided is truly comprehensive.
