Consistent 20-25% annual turnover is never a good sign - Permeating "not my job" attitude and atmosphere - Supervisors and Managers keep all the acclaim; employees do the actual work/presentation and pass full reports. Improbable ratio of management staff win Employee of the Month which earns a trip to Florida (and it's all due to tasks team members accomplish, not because of actual superstar supervisors).
