Needed materials/ equipment are never available. Deans who are either indifferent, or too distracted to respond to instructors' needs. They let departments that have enrollment problems wither, instead of looking at necessary innovations, like outreach. Department head who is openly hostile to students and faculty alike. Questionable hiring practices that do not take preferred hiring into account.
