Opportunities for professional growth and advancement are limited, particularly for employees who remain with the organization long term. Career pathways and promotion criteria were not always clear or applied consistently, which made advancement challenging. Leadership decision-making did not consistently incorporate input from internal subject-matter experts, which at times slowed progress and limited the effectiveness of improvement efforts. Accountability at higher levels of leadership appeared uneven, while expectations remained high for frontline and mid-level staff.
