Applied with Easy Apply on LinkedIn. Received an email addressed "Dear Candidate" inquiring about my sustained interest. Then they sent a 15-item questionnaire. My next correspondence was a job offer assuming I had accepted it, with clearly copy/pasted sections in a form email. When I replied saying I was skeptical, I never heard back.
