Mistakenly sent a rejection form email even though I was already told I would advance and had to ask why for her to realize her mistake. Eventually after several rounds received yet another boilerplate rejection email (very crass - especially after so many rounds). Had to ask again for feedback (since there was a track record of disorganization and mismanagement of candidate information).
