Typical large corporation confusion. Lack of clear company vision at worker level. Too much energy spent putting out fires versus proactively preventing them. Wheels of change can be very slow with regard to updating procedures, processes and methods. Company systems are comprehensive, but utterly un-intuitive with next to useless users manuals.
