Poor communication between many unorganized department leaders interested in mostly guarding their turf and advancing their own careers with little interest in doing the right thing for the organization. Many, many "IT leaders" with no real expertise in their subject matter. EVERY major decision a political decision. No real accountability, if you see this as a problem.
