Leadership is inexperienced, and have questionable ethics. Lots of broken promises for advancement and compensation. People are constantly working under the stress of being let go at any time for no reason other than "role elimination". Compensation is below industry averages. Leadership doesn't truly understand the roles they promote and hire for. When people start they change the responsibilities and/or expect people to accept sudden changes, added responsibilities and zero additional compensation. It really just sets people up for failure. The company is a rotating door of employees - why else would they need a recruiter when head count never actually increases.
