Weak leadership. Ambiguous or entirely unwritten job descriptions. Focus on rapid growth that outpaces the companies abilities to maintain culture. The founder states a culture focused on caring for employees but that does not translate to some of the middle management who blatantly lack even a basic ability to lead.
