Leadership frequently made decisions to meet short term goals or to chase an attractive client at the cost of investment in foundational infrastructure. They could always push off a necessary backend update because a human could manually do it in the meantime, until that human is burnt out from doing the job of 3 people and barely getting a competitive salary for 1 of the roles performed.
