Sweeping SOP changes are levied impacting multiple departments without communication between teams. New ideas are often derived from niche forums and unrelated business models. The internal communication between departments is often needlessly hostile. A direct result of poor leadership, seeking blame instead of solutions. Some individuals will knowingly not execute their role and seek a scapegoat to avoid blame or culpability when things go awry.
