Too much focus on process. Need to remove the politics out of the portfolio process. Not enough focus on real investment in the IT employees. Way too many hours on a constant basis. The 40 hour freebie skill soft training on an annual basis is not sufficient to keep with the ever changing world of IT. Be open to rotating people on a regular basis, so that they gain new experiences, build their knowledge, and become a better cross-functional employee. Moving USA based jobs to Prague and Singapore in order to reduce labor costs.
