Lots of "we are going to do it this way because that's the way it's always been done attitude". The IT Department holds the business hostage with its reports. No real business information. Even data is difficult, if not impossible to obtain. No Data Warehouse! How the heck do these people make decisions? The finance department abdicates most of what you would think were their accountabilities and instead hold operations responsible. Middle management have their direction focused by the CEO on trivial things instead of important business basics. It's like the house is on fire and the middle managers want to rush in to save the kids and instead they are being asked to urinate on the garage. There is some blatant nepotism where some completely unqualified lower management have been hired and immediately promoted. Then called heroes for implementing ridiculous business practices.
