The problem with the organisation can stem from the following: 1. Lack of a coherent strategy 2. Very heriarchial, and no listening to what is happening in the ground. Information is held and not actively shared and develops a a lack of trust (even though it may not have been intented). 3. A toxic culture which leads to quite high turnover through brownout.
4. There is a perception that from the top, management do not trust staff or expertise. Challenging thoughts is not allowed even if these are with the best of intentions.
