-Weak engineering leadership on all levels -Chaotic engineering departments with little to no standards, process, or quality control. -Inexperienced and untrained engineering supervisors with deficiencies in communication skills and low internal locus of control. -Single IT manager for 100+ employees. IT department has debilitating constraints imposed by corporate IT policy and is very slow to act, often rudely. -Poor production leadership with assemblers and technicians not receiving high-quality instruction and information. -Little to no semblance of ownership for quality of work and outcomes. Always passing the buck or blaming someone else, another company, or another person.
