Managers tried to make things right and fair but hands were tied when it came to executing on decisions (eg grades and salaries for people) -Lack of overall vision and communication. Often two different project teams were duplicating efforts by working on the same project or working on projects with completely contradicting goals. -Facility- complaints raised about ineffective HVAC (building either freezing or roasting), smoking area on balconies near fuel zones do not even meet health and safety code -Reward people who grow up at the company; external hires make up to 25% more than leaders who are grown from within
