Management, or lack of - absolutely no leadership! Usually, the revolving door applies to lower level workers, but it seems supervisors and managers are the guys who can't last. People who have been there the longest are the worst at their jobs, and the least knowledgeable. "It's not my job" and "no one told me" are common excuses. As for the people who actually try and put forth effort, they're often seen as a problem, and every effort will be made to get rid of them.
