      1. Management has mental block for taking decisions. 2. Management agree for the issues and problem but do not act. (Point 1) 3. CEO do not believe in tools, simulations, project management/schedule/time lines, organization etc. 4. CEO commit to employees that he will close the task in given time but never hold to commitment. 5. Extremely demotivated teams.
