Not enough resources working on architecture. Those that are certainly are very, very senior, but simply aren't bold enough, or don't seem to realise the pain that everyone is experiencing due to a product architecture that isn't keeping up with the company's fantastic growth. Most of the tech management has little idea how to improve SDLC processes. There's a significant head-in-the-sand culture. The tough decisions just don't get made.
