Organization undergoing major changes with no clear plan. They change their organisation structure twice a year. Very difficult to make progress on anything. The business is doing very poorly, the organisation is struggling to cope with basic processes and has no resources to invest. Lacks technical individuals in key functions, very unlikely to learn best practice from your colleagues, no budget to attract talent or train employees. Very hierarchical, most senior managers are veterans with the company, but in general outdated.
