Lack of leadership from upper management and their weird obsession with having employees working unpaid OT. They are also too distracted trying to chase new business opportunities and bigger clients that they don't have the resources to handle instead of maintaining good relationships with existing clients.
Also, they're unreasonably stingy when it comes to budgeting: expects employees to pull diamonds out of thin air. And even more unreasonable when it comes to talent/resource management: gets upset and disappointed when an entry-level employee cannot deliver a project that is should have been done by a senior art director.
Promised benefits such as insurance never came.
There's a hint of nepotism also: one of the art directors is the founder's wife and no one shall dare cross her.
The overall attitude of the people is really toxic; work with them long enough and you'll become one of them. That explains the high staff turnover.

Casual and easygoing environment. There are hardly any outright conflicts or confrontations as no one seems to be invested enough to care what happens next.

Unbalanced assignment of resources and poor work management. Lack of suitable financial compensation even when work has been exemplary. Little to no business/creative guidance or involvement by management, who seem to be fixated on their other sources of income to bother about the future of the agency and its current client pool.
